A plumbing contractor died after an unprotected trench he was working in collapsed. The victim was an independent plumber subcontracted to install a sewer line connection to the sewer main for a single-family residence.

A master stonemason died after falling roughly 30 feet to the ground from a wooden two-tiered work platform. The platform, which had been fabricated by the victim and another employee, was fastened to the forks of a powered industrial truck. At the time of the incident, the victim was preparing to exit the platform, and the front guardrails on the upper tier of the platform were not in place.

A 35-year-old material handler was killed after she was crushed against a podium by a backing forklift. The forklift was in good operating condition and the back-up alarm was working at the time of the incident.

A 47-year-old auto mechanic died when gasoline he was pouring into a motor vehicle ignited and burned him. The victim had worked at the auto shop for two years. His employer did not have a written safety program, Injury and Illness Prevention Program, or job training program that provided safety training to employees.

A 50-year-old janitor was found dead in an office building where he was cleaning floors with a propane-powered buffer.

A 56-year-old warehouse worker was killed after he fell approximately 30 feet through a skylight and onto a concrete floor. The victim had been employed at the warehouse for about seven years and was the chairperson for the facility’s safety committee.

A millwright was killed while welding additional metal to the feed rolls of a debarker at a lumber mill.

A 67-year-old machine operator died from burns when a fire ignited between the engine and cab of a bulldozer. The victim had been working at the company as a full-time heavy equipment operator for about 30 years.

A 33-year-old equipment operator was killed after he was pulled beneath a 45-ton bulldozer. The victim worked for a company that employed about five full-time workers and hired approximately 70 additional contract laborers.

A 45-year-old employee of a ready-mixed cement company was fatally injured when he was backed over by a skid-steer loader.
