Alcohol and drug test statement
This “Previous Pre-Employment Employee Alcohol & Drug Test Statement” form can be used to comply with 49 CFR 40.25(j), which requires employers to ask workers applying for a Department of Transportation safety-sensitive position whether they’ve tested positive or refused a DOT pre-employment test and weren’t hired within the past two years. This helps identify applicants or new hires with an unresolved DOT testing violation or outstanding DOT follow-up tests. The form is available in a pad format; each pad contains 50 forms (measures 8½ inches by 11 inches).
