$336,200 to a Massachusetts construction contractor for fall protection violations in connection with a worker injury
$137,400 to a Missouri manufacturer for lockout/tagout violations related to a worker’s finger amputation
$81,000 to a Delaware plastics company for machine guarding violations following multiple employee injuries
$69,200 to a steel erection and roofing contractor in Alabama for alleged fall hazards related to injuries suffered by three employees working outdoors during a thunderstorm
$33,000 to a waste management company and a temporary labor provider in Texas for the heat-related death of a worker

The Office of Information and Regulatory Affairs – part of the White House’s Office of Management and Budget – reviews proposed regulations. The process is required for most rules before they can move forward, and typically takes 90 days.
