$303,900 to an Illinois-based precast concrete products manufacturer in connection with the crushing death of an employee who was working alone in a concrete mixer’s discharge mud hopper
$228,600 to a rehabilitation program in Florida for a variety of alleged hazards, including exposing workers to amputation hazards and failing to train employees
$150,150 to an Illinois sewer and water contractor for trenching violations that include failing to provide cave-in protection and failing to train employees on trenching and excavation hazards
$105,600 to a Chicago engineering company for alleged trenching hazards, including having employees working in an unprotected trench greater than 8 feet deep
$69,000 to an Ohio manufacturer for machine guarding and lockout/tagout violations following an employee’s death in an unprotected spindle winder
$42,300 to a construction company in American Samoa in connection with an employee electrocution during a crane operation
